What is the Healthy Living Referral Programme?
Sevenoaks District Council provides a Healthy Living Referral Programme, in partnership with NHS West Kent. The programme aims to simplify and co-ordinate referrals, to healthy living, weight management and falls prevention initiatives, by GPs, other health professionals and care providers. The primary objective of this programme is to raise awareness of healthy living through targeted initiatives. These initiatives are available and accessible for all including adults and children, throughout the Sevenoaks District.
The Programme is designed to keep people interested and engaged to sustain positive lifestyle and weight changes long term. The Programme ensures that people are referred into the most relevant initiative for them through personal support from the Get Sorted Team, trained and qualified exercise instructors and nutritionists.

How do I refer?
Step 1
Confirm your patient is ready to change and willing to access one of our healthy living initiatives.
Step 2
Complete the electronic Healthy Living Referral Form (GP/Health professional use) (
WordPad Document 58KB file details), indicating which initiative you would like to refer your patient to.
Step 3
E-mail the completed form to healthyliving@sevenoaks.gov.uk, alternatively print and return by post in an envelope marked ‘Private & Confidential’ to: Healthy Living Team, Community Development, Sevenoaks District Council, Council Offices, Argyle Road, Sevenoaks, Kent TN13 1HG
Step 4
Inform your patient that a member of the Healthy Living Team will contact them by telephone, email or post to discuss their options and a start date to the relevant initiative.
The referral will receive patient feedback, at appropriate intervals.

What happens next?
After referral your patient will be contacted by one of our Healthy Living Team to discuss their options and find a pathway of care suitable for their lifestyle and their particular needs.
Our Team are trained to select the appropriate initiative and can offer information and support in relation to weight, nutrition, cookery and active living. The Team will provide ongoing support to your patient and will let you know of any relevant progress relating to your patient.
