06 January 2009Below is a list of application forms you will need to obtain your Gambling Act 2005 licence or permit.
Under the heading of each licence type are several links which you can use to download the relevant forms.
To use PDF forms: Print off relevant form and fill in manually
To use Word forms: Select relevant form and save to a file on your personal computer. This form can be filled in electronically and then printed off, signed and returned to the licensing team.
